POSTOPERATIVE FOLLOWUP

HISTORY: The patient is a 41-year-old woman who had gastric bypass surgery on 01/05/2005 for the treatment of her extreme obesity. She has done remarkably well. She reports no problems or issues at all. Therefore, no nausea, vomiting, dumping, no bowel problems. She is taking her vitamin B12 daily, and this has resulted in a high level of B12 in her serum, and I have advised her to go back to twice a week on B12. Other than this she has no problems. She has great energy. She is exercising; walking 3 miles a day, has good early satiety and has had mild hair loss.

PHYSICAL EXAMINATION: Her BMI is 31 with a weight of 185 pounds. Blood pressure is 102/80. Pulse 82, respirations 14. She has lost a total of 85 pounds in our program. She looks absolutely well. Her abdomen is flat, soft, nontender. Her wounds have healed well.

IMPRESSION AND PLAN: The patient has done very well after the gastric bypass surgery. She is taking only Wellbutrin as a prescription medicine. She is going to decrease her B12 intake, continue her vitamin B12 supplement, and I have advised her to exercise more in order to achieve the maximum results possible. She will come back to see us in our clinic in 3 months.
