PAST MEDICAL HISTORY: Prostate cancer (treated with seed implantation) status post left inguinal herniorrhaphy and hemorrhoid surgery.

MEDICATIONS: None.

SOCIAL HISTORY: The patient acknowledges tobacco and alcohol use.

FAMILY HISTORY: Negative for GI malignancies, inflammatory bowel disease, or peptic ulcer disease.

REVIEW OF SYSTEMS: The patient denies nausea, vomiting, abdominal pain, fevers, chills, melena, bright red blood per rectum, constipation, diarrhea, unintentional weight loss, or loss of appetite.

PHYSICAL EXAMINATION: The examination was carried out in both the sitting and supine position. There is about a 1.5 to 2-cm pigmented lesion in the lateral aspect of the left breast at about 4 o’clock. This is approximately two fingerbreadths lateral to the alveolar margin. The area is not tender. There is no drainage. There is no actual mass appreciated. Even when there is drainage, there is no bloody drainage from this area. There were no breast masses in either breast. The nipples are normal and there is no evidence of axillary adenopathy given the limitations of examination in this patient’s body habitus.
