HISTORY: The patient is a 34-year-old man who had gastric bypass done on 01/03/2006 for extreme obesity. At that time, he weighed 255.6 pounds. Today, he weighs 172 pounds. His BMI is 28.7. He has no problems. No nausea or vomiting, no dumping or reflux, no constipation or diarrhea. He has good early satiety, good energy. He walks for exercise and plays with his children.

PHYSICAL EXAMINATION: BMI 29.7, pulse 64, respirations 12, temperature 36.5. He appears absolutely well. The abdomen is flat, soft and nontender with a small amount of excess skin in the midabdomen. Wounds have healed well.

IMPRESSION: The patient is doing very well after gastric bypass surgery. His weight loss will slow down in the coming 6 months. I have advised him to pick up the exercise somewhat in order to maintain good weight loss. We will do our routine labs, which include a CBC, comprehensive metabolic panel, and vitamin studies at this juncture. I have advised him to come back for a recheck in 1 year from surgery.
