HISTORY OF PRESENT ILLNESS: The patient is a 36-year-old woman who had laparoscopic gastric bypass surgery on 11/02/2005. She weighed 274 pounds at the first visit and 254 pounds at surgery. She now weighs 162 pounds and has lost 65% of her excess weight. Her BMI is now 31. She has no problems. She is taking a stage III diet well, has never had nausea or vomiting. She has no abdominal pain. She moves her bowels well and urinates well. She is exercising twice a day, doing aerobics in the morning and bicycling in the evening. She has excellent early satiety, is taking her vitamins as directed and has good protein intake. She has had no hair loss.
PHYSICAL EXAMINATION: Her temperature is 37.5, pulse 72, respirations 12. She appears absolutely well. She has pink conjunctivae, therefore, there are no signs of anemia. Her abdomen is flat, soft and nontender. She is now a size 12 dress, which was her goal. Her scars are healed. She looks in very good shape comparing prior to her surgery.
PLAN: I have advised her to continue doing what she is doing. She needs to return to our office in three months. I have ordered routine six-month postoperative labs on her.
